My name is Roger Christiansen I am a 47 year old Diabetic. I was diagnosed about 6 years ago with type 2 diabetes and from the start it was considered a severe case. The last two years have been by far the worst. I have been in and out of hospitals many times, all for complications due to diabetes. I have had lots of infections resulting in several toes on both feet being amputated. The most recent, and by far the most serious, was the amputation of my left leg below the knee on Dec. 23, 2009 at Queens hospital in Oahu. I have fallen three times since then. The last fall on Jan 24, 2010 was the most serious, requiring the stump to be stitched up and re-stapled. Since then I was able to have a stump protector made from H.O.P.E in Honokaa on the Big Island. This has made me feel much safer. But the bad news is I that yesterday I found out that I now have the MRSA infection and Pseudomonas aeruginosa . My wound care specialist, Maria Oka, at Ali'i Health, is trying to get me a wound vacuum and a Pik line so I can get IV antibiotics at home. Here is where the trouble starts. I recently was approved for S.S.I . I was then informed by my case worker that since I was approved, my insurance would change from HMSA QUEST to Evercare. I never had any problems with Quest all my Dr.s accepted it and it was always accepted at the pharmacy. I cant even remember when it wasn’t accepted. Yesterday I was informed by my wound specialist at Ali'i Health (808)323-2110 that they cannot accept Evercare. That means no wound vac or Pic Line. Also I cannot get the antibiotics that I need desperatly. Then Kevin T. McGill from Hawaiian Orthotics Prosthetics Enterprises (808) 775-0814 informed me that he would not accept Evercare. This means NO PROSTHETIC LEG! Which of course means I don't get to walk again. This one really makes me feel sick inside. Then I stopped by the pharmacy to get some test strips for my Glucose meter and the Pharmacist says they do not accept Evercare. I asked how much for the strips if I was to pay cash and they said $115.00 for less than 1 month worth of strips. I have an appointment with my Primary Care Doctor, Dr. Stephen Andl, this afternoon. I doubt if they are going to accept the Evercare coverage either this really is a matter of life and death and time is very important. My question is would it be possible to go back to HMSA QUEST . I need an answer a.s.a.p people die every day from these exact infections that I currently have. I don’t think having a decent prosthetic leg is too much to ask for either. I have also been diagnosed with severe depression and most of that is because of my declining health and all the problems that go along with that. | I am looking forward to the new SHAKA system. I was reviewing the CS52 screen in CPSS and wanted to suggest a face page for the the pertinent people in a case such as DOE, DOH, courts, CWS, etc. along with a way it can be updated or a history can be shown. Since our families tend to be transient I thought it would be great to date the changes to such info as address, phone numbers and so forth. Thanks, Kristi | ||

| Keiki Korner is a cute name and since this will be accessed by our keiki, I believe we should support correct spelling when possible. "Korner" should be spelled "corner." Perhaps Children's Corner or Keiki Corner. For future trainings, staff will probably benefit from seeing what the new screens will look like for their logs, their dispositions, the payment screens, etc. Visually seeing where we are now in terms of CPSS screens and where we will be in terms of the SHAKA screens, will be most helpful. | ||

| Also need a screen where workers input attempts to do FIC's and attempts to find families. Workers go out right away on an investigation and then cannot locate family or children. | ||

| "Since BESSD is developing a portal for their HANA project, why not try to add the Facilities for Foster Homes too. The HANA system allows their Child Care Organizations to view and update some of their information. This would free your staff from having to start from ground zero to develop this capabilityv " | ||

| "Aloha John, Mindy from our Project was at Maui CWS and asked me to pass this on to you. The Child Welfare Services workers would like to be able to put in more than one contact code for the narratives they enter. Currently they can only put in one type of contact code (Example: WVC-worker visit with child or HVS-home visit, scheduled.) They would like to put in more than one if more than one type of contact is met, like WVC and also a code for foster parent home visit, etc. They would like to be able to write the narrative once with two or more codes. | "re: name for the system: it is important the the name of this system be in the Hawaiian language and have proper meaning. In order to do that, the formal name may be longer than you wish, but it would reflect the proper focus of the new system, to protect the children of hawaii. Therefore, after consulting with Hawaiian language teachers, I submit the following: Ke'ena Ho'omalu i Na Kamali'i o Hawai'i this means the Office/Department for the Protection of the children of Hawaii. The short version would be KHKH (Can't make a word out of the words. however, could go by the short name of Ho'omalu, which means protection.v " | ||

Our new CPSS system would be more user friendly if it has spell check and does not back us out of the system. We at intake spend wasted our cutting and pasting from word to CSPP, only to get backed out before we F11. Mahalo for your hard work. " | ||
